Abstract: Spinal cord injury, stroke and amputations are clinical conditions that cause long-term disability by causing paralysis with significant loss of motor, sensory and autonomic function. The use of electrical neurostimulation has been widely proposed to restore function. The use of safe neurostimulation parameters remains a priority. As a result, we developed the first machine learning algorithm to predict neurostimulation-induced tissue damage with high accuracy, surpassing traditional methods. Our model incorporates a wide range of stimulation parameters, facilitating safer and more informed decision-making. Here we present two neurostimulation applications to restore function in a rodent model. First, we used intracortical microstimulation to evoke somatosensory perception of the front limb by implanting a microelectrode array into the somatosensory cortex. We then developed a behavioral paradigm to investigate perception thresholds with different parameters and the spatial limits of these stimulation-evoked sensations. Second, we developed a strategy of pairing vagus nerve stimulation with rehabilitation of bladder function as a potential novel therapy for neurogenic urinary dysfunction. This approach aims to promote neuroplastic changes in spared neural pathways, strengthening neural control of bladder function and offering a novel perspective on restoring autonomic function after injury.
