In December, health insurance carriers sent health insurance ID cards for 2012 to participating employees. If you have not received your ID card, contact Benefits Administration at 368.6781 or benefits@case.edu.
Employees participating in the School of Dental Medicine dental insurance plans or in the VSP vision insurance plan will not receive ID cards.
