For all Benelect medical plan participants (and those adding coverage for 2014) who would like to obtain the $25 monthly premium incentive associated with completion of these wellness activities, there are three upcoming deadlines.
The deadline to complete is Saturday, Nov. 30.
The deadline to complete The Tobacco Attestation Form is Saturday, Nov. 30. This can be found by visiting , logging in, going to Self-Service, then Benefits, then Wellness Program.
Biometric Screenings (blood pressure, height, weight, cholesterol, and glucose via venipuncture) at a Quest Patient Service Center can still be scheduled online at (use key code “CWRU”). The onsite screening schedule is now closed for registration. All onsite and Patient Service Center visits must be completed by Dec. 6.
Completion of all three of these wellness activities is necessary to obtain the $25 monthly premium benefit.
